Recovery Plus Home Visit Consent & Liability Waiver

This document outlines the terms and conditions for receiving recovery services from Recovery Plus during a home visit. By reading this and checking the box at the end, you confirm your full understanding and agreement with the following statements and give your informed consent.

1. Description of Services

Recovery Plus may provide athletic recovery and wellness services during a home visit. These services may include, but are not limited to:

  • Compression therapy

  • Percussion massage using recovery tools

  • Guided stretching or mobility work

  • Use of recovery boots or other recovery equipment

  • Non-medical suggestions related to hydration and wellness

Services may be complimentary or paid. Payment for paid sessions is due at the time of service.

2. Acknowledgment of Risk

I understand that receiving physical recovery services may involve light physical pressure, movement, or equipment use. As with any physical activity, there is a possibility of experiencing soreness, minor bruising, or discomfort. I accept these risks and understand that Recovery Plus is not a licensed medical provider and does not diagnose or treat medical conditions.

3. In-Home Consent and Cleanliness Expectations

I give Recovery Plus permission to enter my home to deliver recovery services. I agree to provide a clean, safe, and respectful environment for the session.

Recovery Plus reserves the sole right to evaluate the condition of the service environment. If a representative of Recovery Plus determines that the space is unsafe, unsanitary, or poses a threat to personal safety or professional integrity, services may be refused or discontinued immediately at our sole discretion. This includes, but is not limited to:

  • Aggressive pets

  • Hostile behavior from anyone present

  • Hazardous or cluttered environments

  • Unsanitary conditions (including unclean floors, surfaces, or strong odors)

  • Any situation that makes our partner feel physically or emotionally unsafe

No refund or compensation is guaranteed if service is refused under these conditions.

4. Property Damage Disclaimer

I understand and agree that Recovery Plus and its representatives are not responsible for any unintentional damage to my property, including but not limited to furniture, flooring, walls, or personal items, that may occur during the normal course of delivering services. While every reasonable effort will be made to avoid damage, I release Recovery Plus from any liability unless the damage is the result of gross negligence or intentional misconduct.

5. Personal Responsibility for Environment

I agree that I am responsible for the condition of my home and any circumstances that may create risk, including but not limited to:

  • Loose or aggressive pets

  • Slippery floors or wet areas

  • Fragile or high-value personal items

  • Crowded, dirty, or disorganized spaces

  • Any individuals present who may interfere with the session

I understand that Recovery Plus will not be held liable for any incidents caused by these factors.

6. Health and Medical Disclosure

I confirm that I have disclosed all relevant medical conditions, allergies, or injuries before the session. I understand that services provided are not a substitute for medical care, and I take full responsibility for my physical response to the treatment.

7. Release of Liability

I release Recovery Plus and its team from any liability related to physical discomfort, injury, or health issues that may result from the services provided. I also release Recovery Plus from any claims of loss or theft unless there is clear evidence of misconduct.

8. Cancellation Policy

I agree to give at least 4 hours' notice for any cancellations or changes to appointments. I understand that failure to do so may result in a cancellation fee or loss of eligibility for complimentary sessions.

9. Media Release (Optional)

I understand that promotional photos or videos may occasionally be taken. I will be asked for explicit consent before any media of me is captured or shared.
